REGISTER WITH THE PRACTICE

The practice are unable to accept registration applicationss from patients if our practice list is currently ‘closed’.

OUR PRACTICE LIST IS PARTIALLY OPEN AND WE ARE LIMITED TO THE NUMBER OF PATIENTS WE CAN REGISTER EACH MONTH.  Please note - if you are currently registered with a GP in the Hessle catchment area we are unable to accept your application until our list is fully open.

REGISTRATION PROCESS

We accept new patients who reside within our catchment area.  Our catchment area is the west of the Hull which includes the villages and townships of Hessle, Anlaby, Anlaby Common, Kirk Ella, Swanland, North Ferriby, Welton and Melton. 

We also accept Out-of-Area Registration applications which will be considered on individual circumstances and it is at the discretion of the practice whether we accept a patient as an out-of-area registered patient. (Please note: we are currently not accepting out-of-area applications during the current pandemic).

 

To register as a patient the following application forms must be completed (per applicant):-

ADULT APPLICATION

▪ Family doctor registration form (GMS1)

▪ New patient registration form (Adult)

CHILD APPLICATION

▪ Family doctor registration form (GMS1)

▪ New patient registration form (Child)

All sections must be completed in full; if any entries or forms or proof of ID are missing your application will be rejected.

We also need provide proof of identity and proof of address to complete your registration application.

A combination of the following will be accepted as proof of identity and proof of address (it is preferable that one item of photo ID is provided along with one document containing your address).

·         Passport

·         Driving licence

·         Identity card

·         Birth certificate

·         Marriage certificate

·         Medical card

·         National Insurance number card

·         Paid utility bills

·         Bank/building society statements

·         Letter from Benefits Agency

·         Tenancy agreement

·         P45/P60

·         Papers from the home office

·         Payslip

Registration – Refusals

The practice are unable to accept registrations from patients if the list is currently ‘closed’.

Under 16’s must register with a parent or guardian.

The practice may on other rare occasions refuse to accept registrations but will not do so on grounds that relate to the applicant’s race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Patients who have been removed from the practice list as a result of abuse or aggressive behavior or have been removed or have left because of a breakdown in relationships, between the GP and patient will not be re-registered.

Patients who are refused registration should contact NHS England, PO Box 16738, Redditch, B97 9PT or telephone the Customer Contact Centre on 0300 311 22 33 or email: england.contactus@nhs.net for help with finding an alternative practice.

Out-of-Area Registration (Please note: we are currently not accepting out-of-area applications during the current pandemic).

Patients who reside outside our practice area may request to register. It is at the discretion of the practice whether we accept a patient as an out-of-area registered patient. Each patient will be considered on their individual circumstances.

The practice will not provide home visits or other urgent care to the patient when outside the practice area and the patient cannot reasonably be expected to attend the practice. If such care is needed the patient would need to ring 111 (NHS 111) to arrange this.

The decision on whether to accept an out-of-area registered patient will be based on the following considerations.

Whether there are clinical conditions or care needs which mean registration without the ability to do home visits would compromise clinical care and the patient’s needs would be better met through registration with a practice near to where they live.

There are practical reasons which mean the patient is unlikely to benefit from out-of-area registration with the practice e.g. the patient is not spending frequent periods of time in or near the practice area where they wish to register.
