Patient Triage - CONSULT WITH US ONLINE

 

You can contact us about a medical, administrative or prescription issue by clicking HERE.

 

Just answer a few short questions and we will get back to you within 2 working days. Please do not use this online form for urgent or emergency requests. Using Online Consultations allows us to quickly and safely decide on the best way to help you, your child, or an adult in your care. The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 1700 - 0900. Please contact 111 during out of hours or 999 if this is an emergency.

 

APPOINTMENTS

Unfortunately we are unable to accept appointment bookings and cancellations via the Patient Triage system.  Please call the surgery on 01482 645295 to make, cancel or reschedule an appointment.

 

PRESCRIPTION REQUESTS

Should be made by using the NHS App, your SystmOnline account or by calling our dedicated prescription line between 10:00-12:30 (Monday – Friday). 

(Please note the prescription line is only for housebound, elderly and those patients who are unable to request via the other routes)

 

TEST RESULT ENQUIRIES

Should be made by calling our dedicated line between 14:00-16:00 (Monday – Friday).   Please select option 2

 

MED3 (Fit Notes)

If you are off work for seven days or fewer you do not need a medical certificate from your doctor. You will however need a self-certification, you can fill out and print the form (SC2) by visiting the following web page: www.gov.uk
